Supporting Information

Evidence (1 of 2)

1. Certificate of Eligibility for GP Registration: Combined Programme Route
CCT (CP)
If you have relevant experience in substantive roles above Foundation level (or equivalent) or from outside of a GMC approved specialty training programme or outside the UK, it may be possible for some of your experience to contribute up to one year towards your GP training programme.  Your previous experience and roles will determine your eligibility for this pathway and the quality of evidence you provide and how this is referenced against the requirements for GP training along with your progress in ST1 will determine the amount of time able to contribute.

For more information about the CCT (CP) pathway, please visit the RCGP website or contact the RCGP’s Specialist Applications team at gpsa@rcgp.org.uk.

Do you intend to apply for the CCT (CP) flexible pathway? (Yes/No)

2. Transportation
You will be expected to be able to attend emergencies and provide domiciliary care which may include out of hours visits if you are based in a GP training practice or in some secondary care posts at any time during your training programme. If you are not a car driver, you are expected to provide a car and driver at your own expense in order to meet this requirement. Relying on public transport, bicycles and taxis is not normally acceptable.

If you do not hold a UK/EEA driving licence but plan to drive on an existing overseas licence, you must ensure that this is valid. The DVLA web site has more information regarding overseas driver licences (https://www.gov.uk/).

Please select the appropriate statements below and accept the statement of intent regarding transport in GP based posts.
  • I hold a full, current UK/EEA driving licence and have access to a vehicle for work purposes
  • I hold a full, current UK/EEA driving licence but do not have access to a vehicle for work purposes
  • I hold an overseas driving licence that currently allows me to drive legally in the UK
  • I do not drive due to medical reasons
  • I do not have a full valid driving licence OR I am taking or considering taking lessons OR I do not drive

STATEMENT OF INTENT
I agree that I am able to provide emergency and domiciliary care at any time during my training programme. If, for any reason, my licence ceases to be valid or I no longer have access to a vehicle, I agree to provide a vehicle or vehicle and driver at my own expense so that I may continue to fulfill this requirement unless, if I am unable to drive for medical reasons, my employer is able to make other reasonable adjustments to the job or to this requirement.
