Orthodontics ST4 Specialty Training Application Form
These are the specialty specific questions used on Oriel for applications to Orthodontics ST4.
The questions are presented here to help you prepare for your future applications. Please be aware application form structures, questions, word counts, etc. can change and there is no guarantee that future application forms will remain the same.
A ‘self-assessment’ topic has been added to the FAQ section on Oriel (https://www.oriel.nhs.uk/Web/FAQContent) to assist you in completing this form.
Listed below are the self-assessment (SA) questions and responses that appear on the application form. Your responses to these questions will determine your score. You will need to support your responses by uploading evidence to the relevant system. The Dental Trainee Recruitment website will give you details of the system and the evidence required. The lead recruiter for the specialty you are applying to will inform you of the timelines for submitting evidence.
Your evidence will be validated by a panel and used to derive your final validated score.
Evidence of false or misleading responses will be collected, and you may be referred to a Probity Panel.

Lack of evidence or unclear evidence will affect your score and may lead to your application being rejected entirely. It is the responsibility of the candidate to provide clear and unambiguous evidence.
Formatting your submission
- Please submit a separate PDF document for each of the questions. Each PDF document must be a single document that includes all relevant pieces of evidence. If more than one PDF document is uploaded per question, only the first will be assessed.
- You should include all evidence relevant to each question in the PDF. If a piece of evidence supports more than one question, you may need to include it multiple times, adding it to the pdf for each relevant question.
- Please ensure that documents not in English are officially translated or they will not be scored. The translator should sign and date their translation and provide their contact details.
- For several of the questions, you will need to provide evidence of LTFT (Less Than Full Time) employment, if relevant to you. This evidence can be the same but needs to be uploaded for each question, e.g. timetable or job plan.
- The panel will only be able to assess the evidence you provide via the system. They will not contact you to request additional evidence or access to any information you have entered on Oriel. You must therefore provide all evidence in a clear format so it can be easily identified and assessed.
- The recruitment office cannot advise you on your responses. You must select the response you feel you will be able to justify to the panel, using the evidence you provide.
- No evidence can be submitted retrospectively.
- Evidence should not contain patient identifiable data.
- Please do not password protect any of your evidence.
Throughout the form, the following asterisks are used to denote: *At the proposed time of post commencement’ means the first Wednesday of September. **All time periods stated refer to Whole Time Equivalent (WTE) of employment excluding any periods of statutory leave.
Any periods of statutory leave should be evidenced and uploaded to the relevant questions.

These should be a minimum length of 30 minutes and could be a small group (at least 2 attendees) tutorial or lecture.
This excludes clinical supervision and should only include teaching sessions delivered to adult learners.

Evidence:
- Please provide collated evaluation from attendees who were present at each of the teaching events, which should include the date, time and title of the teaching session, how long the session lasted in minutes/hours, the target audience and number of attendees
- The collated evaluation must be verified by a senior colleague and evaluations should be mapped to learning outcomes.
- If involved in designing, organising and delivery of a teaching programme, evidence should include letters/emails confirming level of involvement.
- Please do not upload copies of presentations or slides.
At time of application opening, have you achieved any academic publications as a postgraduate relevant to orthodontics within the last 60 calendar months (WTE)*. This excludes published abstracts.
